For much of Tuesday, Senate Democrats had predicted that there would be an early evening vote on the first amendment to the big health care legislation — a proposal by Senator Barbara Mikulski, Democrat of Maryland, to require insurers to provide a package of enhanced health benefits to women, with no co-payments.
